Grant Request Instructions
St. Jude Medical (SJM) Foundation welcomes proposals from qualified organizations. We use an online submission process to objectively evaluate each request. If you wish to request a Foundation Grant, please select the appropriate link below the Grant Request Types heading. To submit a grant request, select the link and you will be taken to our online submission form.
All requests require the following documents to be uploaded:
- Request letter on your organization's letterhead that includes the following information:
- Grant purpose
- Amount requested
- How the grant will be used
- Estimated total budget for the program, activity, event
- W-9 form (Download W-9 from IRS)
Additional documents may be required for certain grant types.

Grant Request Types
Community Outreach Requests
Eligibility: Programs that improve the quality of life of the people and communities. We consider requests that support human services, civic and cultural areas with a specific tie-in to cardiac and chronic pain conditions.

Research Study Requests
Eligibility: Research should be based on satisfying unmet medical needs in the cardiovascular field. Requests should comply with IRS self-dealing prohibitions with regards to St. Jude Medical Foundation's eligibility status.
Requirement: A research protocol, research plan, and/or other documentation describing the research in detail will be required and should include:
- Hypothesis
- Study design
- Significance to the scientific field
- Materials and methods
- Biostatistics, data analysis and clinical assessments
- Enrollment/Sample size and rationale
- Subject inclusion and exclusion criteria
- Estimates of patient population
- Proposed therapy
- Explanation of why the proposed research is likely to advance knowledge in the field and is not duplicative of any prior or concurrent research.
- Description of the required regulatory and institutional approvals for the proposed research status of each.
- Detailed study budget specifying all funding sources including the contribution requested from the St. Jude Medical Foundation, other subsidies and reimbursement from third party payors, as well as identifying cost for conducting and sponsoring the proposed research.
- A copy of the curriculum vitae of the principal investigator and any co-investigators.
- A description of the research site’s research facilities to demonstrate their adequacy for the proposed research.

Mission Trip Requests
Eligibility: 501(c)(3) public charity organizations supporting mission trips outside the United States to perform surgical procedures and provide education and training programs for healthcare professionals in developing countries with limited access to modern healthcare and where lack of training and awareness prevents access to lifesaving therapies and quality health care.

Eligibility: K-12 STEM (Science, Technology, Engineering and Mathematics) or next generation technology programs and educational wellness programs.

General Operating Support Requests
Eligibility: Funds must be applied toward cardiac and chronic pain education and/or advancement of cardiac and chronic pain programs. Funds cannot be used for capital expenditures. We require grantees receiving General Operating Support to submit narrative and financial progress reports near the end of each one-year grant period.
