Important Criteria All Requestors Must Meet
Prior to filling out the grant request form, the requesting organization meets the following conditions:
- Organization must be a public charity as defined under Section 501(c)(3) of the US Internal Revenue Code
- Requested funding is used for purposes related to cardiovascular, neurological and chronic pain conditions

Download and complete the three required forms by selecting the links below. Include a copy of your completed IRS W-9 Form.
Note: You’ll need Adobe Acrobat® Reader to view and print PDF files. Get Adobe Acrobat® Reader. Follow the instructions provided.
To download the document to your computer using Microsoft Internet Explorer:
- Select the link for the document from the list above using the right mouse button
- Select Save Target As... from the menu provided
- Click the Save button.
- The document will be stored on your computer
You can complete the Grant Request Form online if you have Adobe Acrobat 6.0 Professional or Adobe Acrobat 6.0 Standard (or later versions), loaded on your computer. The free Adobe Reader software may allow you to edit and print the form, but you will not be able to save the information and send it via email to the Foundation.
If you do not have the professional or standard version of Adobe Acrobat, print a paper copy of the form and fill out the information using a black or blue pen.
Send Completed Forms
Once you have completed the forms, send the documents as attachments to the email address specified below. Alternatively, you may also fax or mail the documents.
